Zonisamide (ZNS)

1) The main possible mechanism: A combination of multiple mechanisms of action, mainly including the blocking of voltage-gated sodium channels and T-type calcium channels.

2) Therapeutic indications: a broad-spectrum anti-epileptic drug (better for focal seizures).

3) Warning: Patients who are allergic to xanthamide drugs should not use it. Patients with kidney stones should avoid using it as much as possible.

5) Major adverse reactions in children: Including sleepiness, fatigue, ataxia, mental and emotional disorders. Some will have kidney stones and weight loss when used for a long time. Especially when the dose is large, some children will have nausea, difficulty in concentrating, memory and language disorders, and some children willl have decreased sweating (less incidence than topiramate).

6) Interaction with other anti-epileptic drugs: Phenytoin, phenobarbital, carbamazepine and sodium valproate will increase the plasma clearance of zonisamide to a certain extent. In addition, zonisamide can significantly increase the plasma concentration of epoxidized carbamazepine. Furthermore, try to avoid being combined with other antiepileptic drugs that also inhibit carbonic anhydrase enzyme, such as topiramate.

7) Interaction with non-anti-epileptic drugs: Relatively few. Avoid combining with some drugs that are also easy to form kidney stones.
